Signature Put on Top of Each Other

We're having an issue when signatures land on top of each other. A user says everything is working fine until they selected "Click to send another document." Now signatures come out as shown in the attachment. The user's rebooted, signed out and back in, but still see the issue. What are our next steps in troubleshooting?
