Signature stamp with form fields

Hallo together

i have to setup a process where we stamp PDF-Files with a stamp. This stamp would contain several signatures and also some other Form fields. 

Actual, when i just create a dynamic stamp, and stamp my PDF-File i get the grapic but no fields to fill or sign. how to combine this two functions (Stamp with Fields and Sign-Functions)
