Signed Agreement not saving back to custom object in Salesforce

I've tried everything! I cannot get my signed agreement to save back to the custom object I am sending the agreement from. Can someone please help? I see instructions to create a data mapping (I did this and added it to my agreement template). I see instructions to edit custom settings (not sure there is anything I need to edit here...). 

 

It seems like it should be standard functionality to create an agreement record on the object you send the agreement from and then save the signed PDF back to Salesforce files of the record. Did I miss a setting during set up? Thanks!
