Signed Documents not being retunred

I have sent through a document to client 2 weeks ago for him to sign - he has signed but it was never received back.  I sent through a reminder yesterday and met with him this morning - he signed it in front of me and submitted it, but @ 5.30 this afternoon it still hasn't come back as signed - and it is showing up as not signed on the system.

Can anyone tell me what is happening to this document??
