Signed needs to upload attachments to a form

Hi I'm trying to figure out if and how I can have a signed upload a document.

One of our forms basically asks "Did you file a report this month" and the signer clicks a radio button for yes or no.

If yes, they must upload an attachment. Is uploading an attachment possible?
