Simple PDF form with multiple signatures on it that humans in the same room all sign

We have a permit to work form and this is typically something that an external contractor, our Facilities Manager and Maintenance person all need to sign at the same time, whilst standing in the same room.

 

This is currently a paper form in a book that is signed with a pen and has multiple foil tear off carbon copies to hand to each person.

 

I want to use an iPad with a blank PDF permit to work form (either a fillable PDF saved on the iPad or a web form), that the Facilities Manager completes together with the contractor and the maintenance person present, the FM then signs the permit with their finger on the screen, the contractor signs the permit slightly lower down with their finger and adds their email address and then the Maintenance person signs with their finger at the bottom of the form.

 

When this single page is signed by all three right there and then, a copy of the permit is emailed to each person displaying their signatures.


I don't want verified digital signatures and also don't want the form being emailed to each person asking them to sign as this makes the process long winded and more complicated than it needs to be.

 

Can I achieve this with Adobe Acrobat Sign Pro? If so, how.
