Single Document Needing Multiple Signatures

I am trying to send out one document via Adobe sign to 4 different people. Each person needs to sign the document. One of those four also needs to initial, and another one of the four needs to fill in a few text fields. When I click send, it's only been going out to one recipient or the recipients are getting emailed anything. I've followed Adobe's online step-by-step guide and it doesn't seem like I missed anything, but apparently I am.

 

It should be noted that all fields I put in were assigned to the correct people, and fields were changed to Signature/Initial fields where appropriate.
