Template field size and placement changing.

I have a standardized document (timecard) that multiple users will send to their manager for signature. 

I created a template placing the fields for Sender ("Add me" option for self signing) and the Signer (Manager).

The template was designed to place the fields in the right place on the form so users can simply apply the template rather than separately add the fields each week.

When the template is applied, the fields are correctly assigned but their size and placement is not right.

What am I missing?
