Template fields don't line up for a co-worker

I have templates that I share for my co-workers (2 of them) The field line up for me and one co-worker but the 2nd co-worker using the same template and fields don't even come close. We all use the same documents and then add the fields by applying a template. Any help would be greatly appreciated!
