unable to combine a template and document into one file for esignature

I last did this in October, not sure why I am unable to do this now, but here we go:

 

I have a 20-page form that I painstakingly made into a template.  I used to combine it with a customized letter and send one document to be completed and signed.  This no longer works. I have tried beginning with the letter and adding the template, and I have tried beginning with the template and adding the letter (both require a signature), but neither works.  I am now sending two documents for  signature.  Please explain how to do this or why I am no longer able to....

    did you find a solution to this as i have the same issue?

    i used to be able to combine my Esign template with a file to create a single file that my clients could fill and sign, but this functionality seems to have been removed?

     

    does anyone have any information on this?
