Unknown Form Fields Provided

Getting a weird error on a form that has worked for the last year or so. 

 

Error only comes up when a signer is on iPad and selects a box that shows additional required fields. They provide the information in the fields, however it fails to sync from the iPad and complete the workflow.

 

Form has been deployed since 2022 and this is the first time we're getting the error. Any help is apperciated.
