Using the same Sign form multiple times to track document's distribution history

I want to be able to send out a form that will be used to record a document's revision/distribution history. Each time a new version of a document (not this form) is made, that doc is distributed to the relevant parties. The form I'm trying to make gives each party a place to sign/initial, acknowledging that they received that new version of the document.

 

In Adobe Sign, I've so far been able to make a form that can be initialed by parties once and the date field is populated correctly with their signature date, but I need this form to be signed by these same parties multiple times in the future to accomodate for each time the document is updated and distributed. I am having trouble figuring out how to get them to sign the form again.

 

Security (in this application) is a very low concern, but Adobe automatically certifies any form I make once that form is sent to the recepients, rendering it uneditable from that point on.

 

If I'm able to deactivate the auto-certification feature, is Adobe Sign capable of doing what I'm looking for? And if so, how do I go about changing settings to make this form editable and signable in the future?

As the document needs to be signed multiple times by the same recipients, please let us know how often it will be signed.

You may refer to the Workflow feature in Adobe Acrobat Sign: https://helpx.adobe.com/sign/using/workflow-designer-signature-workflow.html. Check if that works for you.

 

In general, the document gets signed multiple times by a recipient only when you add the recipient's email address multiple times to the recipient list in an order you require a signature.
