Weird problems with fields when trying to build forms and request signatures..??

I've got a basic lease agreement with nothing but a Name text field where the person should be able to type their name and a signature field where they can digitally sign.

 

I'm using "prepare a form" to place these fields in Acrobat, but then when I click "request signatures" it loads a new interface and does not properly carry over the fields I had added.  

 

I then re-add / organize the fields on this "request signatures" UI, but then when I actually send for signature it's not carrying over the text field where they should be able to enter their name.

 

I cannot figure out what I'm doing wrong here.  Any information on this would be greatly appreciated.  Thanks!

Thank you for reaching out, and sorry for the trouble caused.

 

We did check the video link shared by you. There seems to be an issue. However, to verify this, we need to replicate the behaviour. Please share the document used here so we can replicate the behaviour. 

 

In the meantime, please try to send the document directly via the Acrobat web account. Open the link https://adobe.ly/3YcSple and sign in with your Adobe credentials. Then go to the E-Sign tab and select Request Signatures. Check if you still experience the same behaviour.

Thanks for getting back to me.  I wound up creating a new doc so I don't have that exact one, but I'm having the same problem with any doc I do this with.

 

What I did learn was that the text/name field got entered automatically when you go ahead and do the digital signature.  Well, based on typing in the signature.  I'm not sure what it would have put there if I used the actual "sign" option.

 

The thing that's getting me, though, is that building out the form in Acrobat and then choosing "request signatures" never moves all of my fields into the new screen successfully.  It always drop some of the fields, and it always puts the wrong "person" linked to the fields that it does bring over.

 

I can get another example if you need, but again, it's happening with every doc I do this with, so I feel like I must be doing something wrong.  Just not sure how or what that could be since it's a pretty straightforward procedure.

 

Any other guidance you can offer would be greatly appreciated. If you do need another specific doc to reproduce with let me know and I'll get you one.  Thanks.
