Why are my text fields duplicating once it's signed?

When I send out a document for signature, I insert signature and text fields by dragging into document. Each field is labeled differently: "Signature 1" "signature 2" etc.

However, once the document is signed and filed, some of the fields show a duplicate signature or text making it hard to read.

Any ideas on how to prevent this?

Are there any fillable fields on the PDF itself (before it is uploaded to Adobe Sign)? I ran into an issue where some fields were being duplicated because they were on the PDF already, in addition to being added manually in Adobe Sign.

This only occurred with text fields and not signature fields, so it may be a separate issue - it may be worth looking into, though.
