Why are the form fields not appearing in the widget/hosted version of the document?

I created a widget of a document, so that I could send to new hires to sign within an email, however when I test the link the form fields are not appearing. Any idea why and how do I fix it?

    Hi colleens85625281,

    Have you added the form fields while creating the widget ?

    Are you able to see those form fields in the widget in your EchoSign account under the Manage tab?

    If yes, try an alternate web browser to test the Widget.
