Why when someone signs a form in Echosign my fillable fields get written over with other text?

My adobe acrobat fillable fields get over-written with junk when I send it in Echosign with signature boxes on the page.  What's with that?  It never did that before.

Can you share the screenshot for that or inbox me the copy of document? Also, were the fields already filled or were placed over the written text in document?

Hello Rijul,

Attached is what it looks like when someone signs it and an example of what it normally looks like before signature. On this particular wire request form, I had to scan the fillable page in order to get the signatures.

This just started happening about two weeks ago.
