Hello Everyone I have a question if I change my first and last name in the contributor profile then should I also submit a new tax form?

Your tax form needs to reflect your real name. It's an official document and not a screen name. That need to be correct.

Though your display name can be whatever you want to be (many Contributors don't use their real names), I believe the account name must be your legal name. Presumably, the name on the tax form must also be your real legal name. In this video, Mat Hayward indicates that it must be your real name.
