Assistance Needed Regarding Model Release Rejections

Question

I have already received 7 rejections related to the model release. I am filling them out in full accordance with all the rules provided on Adobe’s website. I don't understand what the issue might be. Could you please help me figure this out?

Kepler Studio is not an Artist (the person). You need to enter the correct name of the artist which needs to be you and your real name. The ethnicity is not Ukranian, but Caucasian. They do not ask the nationality.

The address needs to be the address where the model lives. It looks to me that you simply added twice the same address. I do not know if the address is complete, but distrubute thenm on 2 lines, you have 2 lines.

I would also fill in the optional part: description and date of the shooting.
