bonjour ,

1-sur le document d'autorisation droit à l'image , le témoin peut il être un membre de la famille de la personne photographiée ou du photographe ?

2-la même autorisation peut elle servir plusieurs fois pour la personne photographiée ou est elle à usage unique avec nouvelle date à chaque fois

A family member can serve as the witness on a release but not the photographer or the model. The witness is signing to confirm they have seen the model and photographer sign the release.

You can use the same release for multiple shoots featuring the same model though I recommend you obtain a new release for your protection each shoot you do.
