Help with "Intellectual Property Refusal"

Help please? I'm trying to understand why this image was rejected "Intellectual Property Refusal".  It had a full Adobe-style release form.  Here's the metadata:

Don't scratch your head. The refusal reason should have been quality issues. You should not apply filters. Let the buyer decide how they use the picture.

It appears that the Moderator selected the wrong rejection reason code. The image is blurry, black and white and contains a frame. Should have been rejected for quality issues.
