Images in both "New" and in "Portfolio"

I had thought that new uploads went to "New" until submitted and then if approved moved (automatically) to my Portfolio.  However have noticed I have a few images that are both in "New" and in my "Portfolio".  It maybe something I have done wrong but is my understanding correct?
