Hello, 

Just wondering is a PO Box address okay to use on a Model release form? In particular, if it is my PO Box address, and i am the self portrait Model?

A self-protrait (selfie) should contain the same address you use on your Adobe Contributor account and Tax forms, yes?

Be consistent.

    The address will be used by Adobe, if there are legal troubles with the model release. So this one should be accurate. That's all. If you are the model and the photographer, it's probably irrelevant. They know how to reach you. 😁
