Model Release always rejected

Hi everyone, I'm a new contributor to adobe stock and my pictures need a model release. I keep trying to correct my model release, following adobe regulations but my submission always on reminder/pending because of model release issue. I already covered personal information of the model, so I would appreciate it if you can help me pointing out what's exactly wrong from my model release. 

I am kinda desperate here because the model release keeps being rejected, please help T_T

ps: the model release I submit to adobe has the model image, for privacy purpose, this one do not have the image.

Try submitting it as a legible JPEG instead of PDF.

Thank you for your suggestion, I already try to submit it with JPG. But it still goes to reminder because of model release T_T

Use your real name and real signature to sign the model release.

Thank you verry much for your input, but my contributor account is using my studio name. Would it be okay if the account name and the artist information doesnt match?

I'm not sure whether they need to match, but you must sign with your legal name.
