my model releases do not accept

Hi, my MEMBER ID is 210177156. I am not accepting model releases, I have tried to add several times and always releases are not accepted. maybe this is a bug, can you look at my profile and explain what's wrong, thank you

I took a look at the rejected release and I'm not seeing anything that would normally trigger a rejection. I will dig deeper into this and let you know what I find out.
