Tax Form Issues

Hello: I've been having issues with the Tax Information section under Contributor Account since 2018. I've emailed adobetax@adobe.com multiple times without response. The only feedback I get is from Adobe Artist Relations asking me to try different browsers. I've tried Chrome, Safari, and Firefox without success. I've even tried different computers. The problem happens when I get to the screen https://contributor.stock.adobe.com/en/account/tax-third-step. When I click the button Complete W-9, a red error box appears at the top: There was an error while opening your Form.

 

I've even emailed my W-9 to Adobe, hoping to bypass this step. I would like to add more content, but what's the point if I can't get past this step

IMPORTANT!  Use a browser that accepts cookies and does NOT have any script blockers.

I've done all these steps multiple times. I've even tried multiple browsers. It turns out that you need to put something in the last name field even if you are a company (company doesn't have a last name). Then it will work, but then the automated response will deny it because the company name doesn't match (because you have to put something in the last name). One step forward, two steps back.
