Issue uploading the final assignment in Presentation Design course on Adobe Digital Academy

I finished all the steps in presentation design course on Adobe Digital Academy and the last step is to submit the final assignment pdf to the upload form and complete the survey. The issue is I am not being shown the upload form on the last page to submit my pdf assignment even though all the steps are checkmarked and I have completed the course survey.

How do I submit my final assignement to complete the course and recieve a badge from Adobe.
