Not all quizz questions come up

When taking a quizz, it skips questions and then tells me I've failed - of course I have - I never got the questions. Example: Last quizz skipped from question 2 to question 6. So results show questions 3, 4, 5 as being incorrect.Tried takit did the same thing again.

    Prime only follows the course design. If you are not getting the questions then we may need to check the entire project to find the issue and fix it.

     

    May I know who created the course?

    Which Authoring tool was used to creat it?
