Quiz analyzer not functioning properly

The quiz analyzer doesn’t appear to be functioning correctly. I took a practice quiz and answered 5 questions correctly. For some reason the quiz report (attached), shows that all 5 questions were skipped.

    Looks like you are using Captivate. This forum is for Captivate Prime. For any Adobe Captivate related query please post it on Captivate forums : Adobe Captivate

    Can you please provide some more details about the issue? What is the version of Captivate you are using ? Are you using Internal Server Reporting in Captivate if Yes which Internal server you are using for Reporting in Captivate? Can you try using WAMP server and check if you are able to reproduce the issue ?
