"Try Prime" missing from Presenter

I created a Quiz in Presenter, but am unable to publish the document to Captive Prime.  The instructions indicate 

Publish -> Adobe Captive Prime -> <select Prime Account> -> Publish

The problem is the list of Prime Accounts is blank.  Instructions indicate either a "Try Prime" or "Setup/Retrieve Account" should be present.

If I try to publish, the pop-up wizard is blank and unresponsive.

I am working with a trial copy of both Presenter and Captive Prime.  Is this a limitation of the trial functionality?   BTW - the Quiz works when published locally to PDF.
