As described the the documents in Adobe Scan has been reshuffled and the names do not match with the documents.
Have you started experiencing this issue recently, or was this issue occurring earlier as well? What is the version of the Adobe Scan app and the OS you are using? Please try updating the app to the latest version available on the App/Play Store and reboot the device once.
Also try accessing the documents on web here https://acrobat.adobe.com/link/documents/scans/ and see if the files appear with the correct file name. This will help indentifiy if the issue is with the app or not.
Also share a small video recording of the steps you are taking and the issue as it occurs for more clarity.
