Document missing

Hi sir

I had been scanning documents for a week straight and the same went on for the 5th day. I had scanned the entire document renamed the file and tried to upoad it in google classroom but the file that was scanned the previous day got uploaded and the newly scanned document goes vanishing without even a trace in the device. how is it possible
