The online toolset is built for this purpose and allows you to do a lot with both interactive video hotspots and audio hotspots.

Nothing to do with an app. At this point this comes exclusively down to proprietary implementations in the players and needs to be authored every time specifically e.g. using the YouTube media tool. There is no universal format for this.
