Can't apply content to screen with tracking data - Mocha Tracking

I'm having the hardest time applying the tracking data to a video file that is supposed to go onto a phone. After using Mocha to track the screen, when I create the tracking data and apply it to a solid or a video, the content moves into the frame correctly but it won't track throughout the rest of the shot. Appreciate any help!
