Error Code 86 :: 1 when importing csv file

It is extremely hit and miss whether the csv file will import properly.  Sometimes when I save the file from Excel it will import fine into AE but the row numbers will seem to be a bit off, in that the 0,0 cell seems to be where the 1,1 cell should be, and the rest of the time I get an import error.

I have attatcehd the error screenshots below along with me Excel export settings and the csv file.
