How do I recreate this effect?

How do I recreate this effect? I'm thinking fractal noise and displacement map, but I'm wondering how the effect is only "masked" in certain areas. Can anyone help?

Duplicate layers, using whatever wobbly effect on a greyscale image as a luma matte, some clipping with adjustments like Curves or Levels. Mylenium
