How to add Metadata Fields

Anyone can explain how can I add custom fields (and groups) to the metadata panels?

For instance: a group named "Photography Data" containing fields named
"Category" (Portrait, Landscape, Nature, Documentary, Fashion, Nudes.. )
"Lighting Setup"
etc..
