There are three buttons on the lower right after making changes to an image: Open (which opens the image in PS), Cancel (pretty self-explanatory), and Done. When you click "Done, that means you are done working on that image and can move onto the next image. The reason why this isn't "Save," is that you have not made any perminent changes to the image. What you do in Adobe Camera Raw (or Lightroom for that matter) is to change how the image is interpreted. All of these changes are saved in the image (if it's DNG) or alongside the image as a sidecar file (XMP) if it's a raw image. JPG images are a bit different in that the data for how the image is changed is contained within the JPG but you can only see those changes if you have ACR. So if you take a JPG, make changes, and then send that image to someone without ACR, they can only see how the image looked BEFORE the changes. To fix that you need to open that image in PSk and then save the changes into a new image.

BTW, if you have a bunch of images that all require the same changes, if you right-click on the image and select Copy Settings (or Command/Control-c) you can then go to other images in ACR and paste those change by the same right-click (or Command/Crontrol-v). In the image below you can also see how to quickly remove any change you made. Because you can delete any changes you made, you really didn't "save" anything.
