Thumbnail does not match that in Camera Raw

I have a few thumbnails that, when opened in Camera Raw, are actually different pictures that appear within that working folder. I've found about 5 of them in the same folder that do this. I believe this is a recent event as I've never noticed this before. Does anyone have any ideas?

Could you share the following details about the issue:

Bridge version
Camera Raw version
System configuration
Is the issue specific to particular files/format?
