What metadata should I enter for original works of art I have photographed?

I am going to photograph a few works of art (quilts) so the artist can make a book of her works. I figure I will fill out the metadata like I usually do for photos I take, including the copyright since the photo and the copyright to the photo are mine. But I believe there are fields I should fill in that provide the artists name, title of work etc. What should I populate?

IPTC Extension has fields specifically for artworks:

 

These fields are useful for clearly describing an artwork separately from the photo documentation of it, but they are not always displayed other software and file browsers, so I would summarize the artwork fields in the IPTC Core Description (Caption). This will ensure that information easily seen by a wide range of applications and users. You could write something that looks like the tag you would see beside an artwork in a museum:

Leonardo da Vinci; Mona Lisa; ca. 1503-1506; oil on poplar; 77 x 54 cm; Musée du Louvre, Paris; INV. 779
