head and arms in Character Animator

I have a three fold problem and I just can't figure it out, I looked at every video I can find on this issue and just can't find what i'm missing, where the problem is.

1) my head is warping and disconnecting from the body

2) the body doesn't move at all 

3) the arms are detaching from the body, I don't know how to close that gap under the arms
