head and sholder wierd attachment problem

in the video as you see head and the left part of shoulder has been attached but the right side not!!!I couldn't understand the reason?

Just following up here (we solved this on Twitter) - by reducing head tilt and position strength we were able to get it working better. But because this character essentially has no neck, drawing in more artwork for the shirt/neck area behind him would help with more extreme bends/tilts.
