I do not understand the English language. Is there an Adobe Premiere program in Arabic that I need? Even if it costs an additional fee, I have no problem with that.

The red arrow is pointing to the "English with Arabic support". This option will allow the user to download a Middle Eastern (ME) version of Adobe applications. The ME applications comes with a full English Interface, however, it provides extra right-to-left options and controls.

The poster asked for an "Arabized" version which doesn't exist any more, Adobe used to release such Arabic interface between the years ~ 1994 – 2013 via a French company named "Winsoft".
