Q. Add space to text line

How do I add space to a line? I am filling out a new patient form. The lines which need more space have a black box w/ a white plus sign. I have tried everything including downloading version 11. Can not seem to get the 'typewritter' or toolbar for editing to enable which would allow me to have the space to enter the information they are requesting. Thank you. I have been at this for four hours. Nothing is working.
