VENDOR ID DEACTIVATION ISSUE

I have a Kobo device that I had authorized under my library card. I have since moved and have a new library card that I use. I have to change the verification for my device or I can not open my checked out books. 

 

When I try to deactivate my previous verification, it asks for Vendor ID and password. I've tried my previous library sign in info AND my Adobe account info and neither work. 

 

Any ideas?
