Cloud License allows 2 activations at a time http://www.adobe.com/legal/licenses-terms.html

-Install on a 2nd computer http://forums.adobe.com/thread/1452292?tstart=0

-Windows or Mac does not matter... 2 on the same operating system, or 1 on each

-Two activations on one account MAY NOT be in use at the same time (note in the above license link)

you don't transfer anything.

you install the cc desktop app from here https://creative.adobe.com/products/creative-cloud  onto your new computer and use it to install your cc programs/trials onto your new computer.

p.s. sign out from your old computer if you no longer need cc on your old computer.  you can do that from your old computer or using a web browser and going here https://www.adobe.com/account.html
