How can I recover my CS6 license after a hard drive crash?

I work in a small corporation, one of my employees suffered a harddrive crash and lost her cs6 installation.

For whatever reason, our IT dept (who insists on handling our licenses and installs) cannot locate her license info.  We paid a significant amount of money for that license, how can she recover?
