Light room software installation after computer crash

Good morning,

My Mac crash this past week. The genius team at apple needed to reformat my hard drive. I am trying to reinstall may license of Light room 5 but the web site tels me that number have been already use.

Who can I reinstall my light room 5 license on my Mac?
