TRANSFER OF LICENSE

I am trying to find out how to transfer my license of Adobe Creative Suite 5.5 to someone I'm selling it to.

It's the full version with installer disks, not the on-line download newest version.

I've got as far as downloading a pdf called "Transfer of License Form.pdf" but following the suggested path www.adobe.com/go/support/contact.

But nothing is clear about the process from then on. Anyone successfully done this?
