where do you submit the transfer of license form?

A friend of mine is giving me their photoshop CS3 program and instructed that I need to complete and turn in the transfer of license form within two weeks of activation of the program. At the bottom of the form it gives instructions to submit the form on this following website:

www.adobe.com/go/support/contact

, however, I am unable to locate where I am supposed to submit the form. Please help!

    What I need help with more specifically is how to obtain a case number. My account does not show any options as to how I can open a case.
