you can purchase a cc subscription (but you don't qualify for upgrade pricing), https://creative.adobe.com/plans

otherwise, you need both your cs5 and cs4 serial numbers.  you have 3 ways to recover your serial numbers.

    1.  if you purchased from or registered with adobe, check your account (using the same adobe id used to purchase/register), , https://accounts.adobe.com/

    2.  if you purchased a hard copy/box/cd etc, Find your serial number

    3.  if an adobe program is currently installed and activated, you can recover the serial number

your cs5 license is for an upgrade version.

when starting your installation your prompted for your cs5 serial number.  when the installer fails to find a qualifying product (ie, previous version like cs4) installed, you will be presented with a dropdown of possible qualifying products.  select the one you own.  you will then be asked to enter the qualifying products serial number.

enter the correct number at the correct time and installation will proceed.
